Welcome to PUC's free Online Application. The process of applying online is simple and straightforward, just follow the steps listed below. If you have any questions, give us a call at the Enrollment Services office. The number in the continental United States is 1-800-862-7080, option 2. If you are calling from outside of the U.S., the number is 1-707-965-6336.

Online Application steps:

  1. Fill out the application. Please provide information for every question that applies to you.
  2. At the end of the application, read the Student Life Statement. If you agree with the statement, click on the button marked Submit. The application will then be submitted to the Enrollment Services office.
  3. You will also need to print out one reference form*. Have the reference form completed and returned. Complete the information box on the inside top of the reference form. Then give the form to any of the following:
    1. Pastor
    2. Principal, dean, teacher
    3. Manager, supervisor
    4. An individual who knows you well, other than a relative

Ask them to complete the form and drop it in the mail.

Reference Form
Give the form to one of the following: pastor, principal, dean, teacher, work supervisor or someone else who knows you well and is not a relative.

NOTE: Central, Northern, Southern, and Southeastern California constituent academy or Hawaiian Mission Academy students need not submit any recommendation forms. Your academy sends us references. If you are applying for a B.S. in Nursing, you need to submit only the recommendations required in your nursing application.

After your application is submitted, you will receive a letter from the Enrollment Services office acknowledging that we have received your application. The letter will also tell you what other documents we will need to complete your application.
