This form provides PUC with your medical history and insurance information.

Health Release Form
This is a universal records release form that can be used with Pacific Union College or your health care provider.

Reference Form
Give the form to one of the following: pastor, principal, dean, teacher, work supervisor or someone else who knows you well and is not a relative.
