Proration Request Form

If you have a class or work schedule conflict, which hinders you from attending FUSION, Colloquy or other worship options, please be sure to fill out a proration sheet in Student Services.

By filling out your class/work schedule on the proration form the office of Student Services will determine whether to grant a reduction in the attendance expectation of the student. Please note, for any off campus work schedule conflict, each student must provide a letter from their employer.
