Please be advised that you are required to show proof of the following.
They must be submitted and on file in the Health Services office
to be an enrolled student:
Physical Exam (within the last year for new students)
TB test within the last year unless you have recently
been out of the country. Also be aware that certain departments have additional
specifications.
Insurance information: it is now required that you present your
insurance card and/or submit copy of the front and back of the
card.
